Welcome. We want your first visit to be easy and unhurried. Here is what to know before you come in.
What to Expect on Your First Visit
Your first visit usually takes 30 to 60 minutes, depending on the type of visit. We will talk through your symptoms and history, examine your feet and ankles, and order imaging if it is helpful. Then we will explain what we find and discuss a plan that fits your goals. You will have time to ask questions.
What to Bring
- A photo ID
- Your insurance card
- A referral, if your plan requires one
- A list of your current medications
- Any relevant imaging or records, if you have them
Insurance We Accept
We accept Medicare, Medicaid, and most major insurance plans. If you have questions about coverage or costs, call us at (989) 790-8009 before your visit and we will help.
How to Prepare
Bring the shoes you spend the most time in, since they help us understand your symptoms. If you use orthotics or braces, bring those too. Comfortable clothing that lets us examine your feet and ankles is helpful.
Sending Your Paperwork Ahead
If we send you intake forms before your visit, you can return the completed forms to securemail@mmfaa.com, or bring them with you. Sending them ahead shortens your check-in. For general questions, write to office@mmfaa.com or call the office.
