Referring is straightforward. Send a referral by fax to (989) 790-5071 or call (989) 790-8009 to coordinate. When possible, please include relevant history, imaging, and insurance information.

What We Communicate Back

You will receive timely notes after your patient is seen, including our assessment, the plan of care, and any follow up. If something needs your attention sooner, we will reach out directly.
